My safety plan

A small, private plan for hard moments. Yours alone — stored only on this device.

Fill in what you can, in any order. Even one box helps. There's no right answer, and you can change it any time. You can print it, save it as a PDF, or show it to someone you trust.

Based on the Stanley-Brown Safety Planning Intervention used by NHS crisis teams. Not a substitute for professional support.

1. Warning signs

Thoughts, feelings, situations or behaviours that tell me a crisis may be coming.

2. Things I can do alone

Activities that have helped me ride out earlier waves, without needing anyone else.

3. Places & people that distract me

Settings or social contact that take my mind off the crisis (no need to disclose).

4. People I can ask for help

People I can tell what's going on. Name and how to reach them.

5. Professionals & agencies I can contact

GP, crisis team, therapist, helplines. Numbers here so I don't have to search in a crisis.

6. Making my environment safer

One small thing I can do to put distance between me and what could harm me. Means restriction is the single most evidence-based crisis intervention.

+ Reasons for living (optional)

Anchors. People, animals, future moments, unfinished things. To re-read when the crisis lies to you.
